HOW TO REPORT
During working hours cases of reportable diseases and conditions should be reported promptly to our agency at (636) 949-7400, ext. 4226. Reporting forms can be obtained from our agency. When the reportable disease represents an emergency requiring immediate public health action, you can reach the Missouri Department of Health duty officer after hours, weekends or holidays at 1-800-392-0272.
