Application Process

APPLICATION PROCESS (IN-PERSON)

To obtain a copy of a birth or death certificate from the state or local health department, a person must present a document containing a photo identification — such as a driver’s license; passport; or school, work or military badge.  If an applicant does not have a photo ID, at least two alternate forms of identification must be presented. These documents must include the applicant’s name, and the name of the company or organization issuing the document.

In-person applications are processed from 8:30 a.m. to 4:30 p.m., Monday through Friday (except holidays and special occasions) at:

APPLICATION PROCESS (BY MAIL)

Effective March 1, 2011, requests for vital records submitted by mail must first be notarized by a notary public.  An application form available on the St. Charles County Department of Community Health and the Environment website or a written request containing the required information must be completed and notarized before it is mailed to the health department.  Payment for mailed requests must be made by personal check or money order.  CASH WILL NOT BE ACCEPTED AS PAYMENT FOR MAILED APPLICATIONS.
