APPLICATION PROCESS

Cost is $15.00 per certified copy of a birth certiicate and $13.00 for their first copy of a death certificate and $10.00 for each additional copy of a death certificate purchased at the same time. Checks are accepted with identification. No credit or debit cards can be accepted.

Birth and death records are not public records. Only family members may be issued birth certificates. Copies of death certificates may be issued to persons with a valid interest in the estate. An application must be completed and signed. Photo ID is required when application is in person.

Certificates may be mailed when applications are received with a check or money order. Fax orders will not be accepted. An applicant may request that an application form be faxed to them, or you can print one now by following the links below.
