WHEN YOU COME IN FOR YOUR WIC CERTIFICATION APPOINTMENT

WHAT SHOULD YOU EXPECT?

  1. Certification appointments take about 1 ½ to 2 hours
  2. To qualify for WIC there are three sets of guidelines that must be met:
    1. Women, Infant, Child Status
    2. Income Level Guidelines
    3. Nutrition or Medical Risk Guidelines
  3. Clerks will verify income, Missouri residency, and woman, infant, child status.
  4. Nurses and Nutritionists assign medical or nutritional risk based on weight, height, hemoglobin level, diet history and provide counseling appropriate to assigned risk. They also make referrals to appropriate organizations or services.
  5. If certified as eligible when you leave you will have:
    • ID folder and checks to take to the store for nutritious foods
    • An appointment for a follow-up nutrition session

WHEN YOU COME IN FOR YOUR WIC CERTIFICATION APPOINTMENT BRING:

  1. Picture identification (ID) of mother or guardian of infant and/or child
  2. Identification of infant and/or child (ex. hospital discharge papers, birth certificate)
  3. Person to be certified
  4. Income information for all household members
  5. Social security card(s) for person being certified and the mother or guardian, if available.
  6. Medicaid or food stamp card(s) if applicable
  7. Immunization records (for children being certified)
  8. Proof of current address
